Life Skills Field Trip: Permission Slip

Dear Parents/Guardian,

We are excited to notify you that we will be going on a field trip to JAFCO Children's Ability Center on Wednesday, March 27, 2024, for an exciting experience! The JAFCO Children's Ability Center is located at 5100 N Nob Hill Rd, Sunrise, FL 33351.

Students will be transported by a transportation service and/or volunteers to and from the destination.

Please fill-up and submit the permission slip below in order to allow your child to attend the upcoming field trip.
